Payments for classes are required in FULL by week 2 of term. Fees still outstanding in week 2 will incur a $50 late fee. Term fees that are carried over to the next term will incur a $50 late fee. Invoices are only sent out on request (not automatically generated). No refunds will be given for change of mind. Pro-rata fees are not available for missed classes due to public holidays or personal reasons. Pro-rata fees will only be available to new students starting from week 4 of term. I agree to the term fee policy (required) INJURY / MEDICAL CONDITION POLICY & CONSENT It is the students / parents responsibility to inform their teacher of any injuries/medical conditions prior to class commencing. Basic First aid is available to every GEMZ student. In the event of an injury GEMZ Dance will contact the emergency contact as listed on this form, and administer First Aid treatment by a qualified first aider. If medical services re necessary and an ambulance required, the student / parent /guardian will incur the costs. Please note physical contact may be required as necessary. I hereby acknowledge that with any physical activity there is a risk of personal injury involved.
